Healthcare for Retirees

Spain's healthcare system is ranked among the best in the world. As a retiree, you have two paths.

Public Healthcare (Seguridad Social)

Not immediately available on an NLV. After obtaining permanent residency (5 years), you can apply for a SIP card and access free public healthcare. UK retirees with an S1 form can access public care from day one.

Cost: Free (after residency) or via S1 form

Private Healthcare

Required for the NLV application. Sanitas and Adeslas are the most popular among expats. Premiums for over-65s range from €150–€300/month. Shorter waiting times, English-speaking doctors, and access to top private hospitals.

Cost: €150–€300/month (65+)

Tax Implications for Retirees

Becoming a Spanish tax resident has consequences. Here is what retirees need to know.

Key Points

Beckham Law — Not for Retirees

The Beckham Law (24% flat rate) is only available to employed or self-employed individuals who move to Spain for work. Retirees on an NLV do not qualify.

IRPF — Progressive Income Tax

Pension income is taxed under Spain's standard IRPF brackets (19%–47%). Your worldwide income is taxable once you spend 183+ days/year in Spain.

Double Taxation Treaties

Spain has treaties with most countries to avoid being taxed twice. UK State Pension is only taxable in Spain (not the UK) once you are Spanish tax resident.

Modelo 720 — Foreign Assets

If you hold overseas assets worth more than €50,000 in any category (bank accounts, investments, property), you must declare them annually by March 31.

Estimated Tax on Pension Income

€15,000/year~€1,300 (effective ~8.7%)
€20,000/year~€2,250 (effective ~11.3%)
€28,800/year~€4,300 (effective ~14.9%)
€40,000/year~€7,600 (effective ~19%)
€60,000/year~€14,100 (effective ~23.5%)

Approximations based on 2026 IRPF brackets. Consult a gestor for your personal situation.

Frequently Asked Questions

Can I access Spanish public healthcare as a retiree?expand_more
If you hold a Non-Lucrative Visa, you are not automatically entitled to public healthcare (Seguridad Social). You must have private insurance for your visa application. However, once you obtain permanent residency (after 5 years), you can apply for a SIP card and access the public system. Many retirees keep private insurance anyway for shorter waiting times.
Will my UK State Pension be paid in Spain?expand_more
Yes. The UK pays State Pensions worldwide and adjusts them annually (unlike some frozen-rate countries). You can receive it into a Spanish bank account. You will need to inform HMRC of your tax residency change and may need to complete a double-taxation form to avoid being taxed twice.
Can I buy property in Spain on a Non-Lucrative Visa?expand_more
Absolutely. There are no restrictions on non-EU nationals buying property in Spain. You will need an NIE (tax ID) and a Spanish bank account. Buying property does not automatically grant residency — you still need the NLV or another visa route.
How long does the Non-Lucrative Visa process take?expand_more
From initial application at your local Spanish consulate, expect 4–8 weeks for approval. You then have 30 days to enter Spain and 30 days after arrival to apply for your TIE (physical residency card). Budget 3–4 months from start to finish.
Is Valencia safe for retirees?expand_more
Valencia is one of the safest large cities in Europe. Violent crime is extremely rare. Petty crime (pickpocketing) exists in tourist hotspots but is uncommon in residential barrios like Eixample or El Pla del Real. The flat terrain and pedestrian-friendly streets make it very accessible for older residents.
